Billing Policy and Insurance

Managed care plans place responsibilities on both physicians and patients. These responsibilities are outlined in your insurance contract. Understanding your role helps prevent delays with referrals and billing.

Appointments

Please call at least 24 hours in advance to cancel or reschedule. A $50 fee applies to no shows and same day cancellations.

Insurance Participation

We accept most insurance plans. Please contact our office to confirm participation.

Referrals

If your plan requires a referral, it is your responsibility to ensure a valid referral is on file before services are rendered. Referrals are typically valid for 30 to 60 days, depending on the carrier. For allergy injections, some carriers allow a global referral for up to one year.

Please request referrals early. Many primary care offices require 3 to 7 days. We accept faxed referrals at 703 313 4989.

Statements, Email and Text Reminders, and Card on File

We will mail statements to the address on file and may send periodic courtesy reminders by email and text regarding balances.

If you have a card on file through Phreesia and a balance under $500 remains due, the balance may be charged periodically to the card on file. A 5-day notice will be provided prior to any charge. To update your payment method or opt out, please contact our office.

Financial Policy

Charges are due at the time of service. Services are provided to the patient, not the insurance company. Coverage varies by plan, and your insurance may deny payment after services are performed. Any remaining balance is the patient’s responsibility.

Delinquent Accounts

Balances over 30 days are considered delinquent. Balances reaching 90 days may be sent to collections.

Payment Methods

Cash, check, Visa, MasterCard, American Express, Discover, CareCredit, and mobile payments including Apple Pay and Google Pay.

Hearing Aids

Hearing aids are due in full at the time of purchase. We do not bill insurance for hearing aid benefits. Financing may be available through CareCredit.

Copayments and Patient Responsibility

Copays are due at each visit. Patients are responsible for deductibles, coinsurance, non covered services, and applicable fees.

Claims Submission

We file claims for participating plans. Your insurer will send an EOB showing what was paid and what your responsibility is. Secondary insurance will be billed as a courtesy when possible.

Medicare

We submit Medicare claims and coordinate secondary billing when applicable. You are responsible for deductibles, coinsurance, copays, and non covered services.

Out of Network Plans

As a courtesy, we may submit claims to out of network plans. Payments may be sent to you. You remain responsible for payment.

Workers’ Compensation

We file as a courtesy. If denied, unsettled, or unpaid within 60 days, the balance becomes the patient’s responsibility.

Financial Arrangements

If you cannot pay in full at the time of service, please contact the billing department in advance. Payment plans are available.

Returned Checks

A $30 fee applies to returned checks.

Additional Testing and Your Visit

For certain appointment types, additional testing may be recommended before or during your ENT visit to help us best evaluate, diagnose, and treat you. This may include an audiology evaluation, such as a hearing test, or an endoscopy, a brief in office scope exam that allows your provider to look inside the nose and throat. These services are billed separately from your office visit and may result in an additional copay, coinsurance, or other patient responsibility based on your insurance plan.

Attention: Please allow extra time for your appointment due to ongoing construction. The parking lot layout has changed. Free valet is available at the main entrance from 8:00 am to 3:30 pm.
